banning hotlinking from SELECTED URLS

Hi, I'm struggling a bit with this, I would appreciate if someone could provide me with the correct code as all mine has not worked.

I'm trying to ban hotlinking to my server from selected websites. That is, particular culprits. Not only that, but replace the intended image with something telling them that image leeching is banned.

So far my efforts have either just plain not worked, or all hotlinkers have had their images replaced, which is not what I want.

Any help would be most gratefully received.
